Patient Forms
On your first visit we will request that you complete our patient forms that
provide us with your health history and information on your condition. These
forms can be found in the New Patient Center. You can print out and complete
the forms prior to your visit to save time if you wish.
History
Next, you will have a consultation with Dr. Gregory to discuss your
health-related problems, concerns, and potential treatment options. This initial
visit is designed for Dr. Gregory to learn more about you, your condition, and
expectations in order to determine how chiropractic care can meet your goals.
Examination
After your history, Dr. Gregory will perform a chiropractic examination
of your problem and your spinal function. Standard neurological, orthopedic, postural, and physical tests will be performed. Naturally, nothing will be done in
our office without your consent.
The purpose of the examination is to determine the causes of your problems
identified during the history.
X-Ray Studies
Depending on your specific condition, we may recommend X-rays or advanced
imaging such as MRI or CT. The information provided will help us
develop the most effective treatment plan for you, and alert us of any serious
conditions.
Report of Findings
Once the information is collected and examinations are performed, Dr. Gregory will give you a detailed report of all findings and answer any questions including:
1. How can you help me?
2. How often do I need to come in?
3. What will my treatment cost?
After reviewing your health history, goals, and examining your spine and
X-rays, Dr. Gregory will discuss recommendations and notify you if your
condition requires care with other providers. Dr. Gregory will work with you
to produce the best treatment plan for the achievement of your goals.
Treatment
At the conclusion of this initial appointment, you can usually choose to
experience your first treatment. This may include spinal adjustments, physical
medicine procedures or modalities, and/or soft tissue massage. Treatment is
interactive, so you can express concerns about the different styles of
treatment anytime. He will usually include a home exercise program of corrective exercises adapted to your specific needs.
Wellness Program
Many patients desire a wellness program to incorporate outside of and beyond symptomatic treatment. Every person is unique, therefore everyone requires a customized wellness plan. If you desire, we will work with you also to create healthy habits and routines for your lifestyle. The purpose of our wellness program is for you to achieve good spinal function, have a healthy diet, exercise, and maintain a positive mental state.
We provide FMS, SFMA and Fall prevention screenings to determine the need for these services. Call to find out what is appropriate.
